Clinical Judgment on the NCLEX: A Six-Step Study Guide

Review the six functions in the NCSBN Clinical Judgment Measurement Model and use a repeatable process for NGN-style questions.

Clinical judgment questions ask you to make sense of information and choose appropriate next steps. The NCSBN Clinical Judgment Measurement Model describes six functions: recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action, and evaluate outcomes. The current NCLEX test plan explains how clinical judgment is included in the exam.

Practice the sequence

When you work through a case, pause at each step:

  1. Recognize cues: Which findings are relevant? What is new or unexpected?
  1. Analyze cues: What could these findings mean together?
  1. Prioritize hypotheses: Which concern needs attention first, and what supports that choice?
  1. Generate solutions: What safe options address the priority concern?
  1. Take action: Which action is appropriate now within the scenario?
  1. Evaluate outcomes: What change would show that the action helped or that the plan needs revision?

Do not turn the steps into a rigid formula. Use the scenario, scope of practice, and client-specific information to guide your reasoning. After answering, explain your choice aloud or in writing. If your reasoning depends on an assumption the question did not provide, revisit the cues.
